Abstract

To shorten the duration of stay in the hospital, and the planning of the time after the hospital discharge and also to enroll the family within this planning is also very important in the effectiveness of the Alzheimer, like of all other diseases. In addition, the individuals of whom were diagnosed with Alzheimer, cannot provide their personal management of diseases and treatments alone by themselves in most of the time. The primary care nurses and the individuals who are most conducive to co-operate with the other members of the health teams in this area. On the sharing of these responsibilities, their circumstances, priorities, advantages, and limitations, that they were in, should be well evaluated.

The Paced model is the patient-centered case management model on the basis. And the name of the model stands for briefly defined and patient-centered: A patient-centered service; The Assessment: The identification and control of the symptoms, The Case management: case management, The Evaluation of Outcomes: The evaluation of the results, The Discharge planning: To discharge planning and monitoring the integrated health care system. The primary concept in this model is nursing and stands out as a case manager and the nurse monitors the patients from the admission to the discharging from the hospital. While fulfilling this role may plan how to use the community resources, and also plans to provide services in conjunction with other relevant groups.

As a result of this adaptation of the model; the care needed patients may receive and maintain a better quality and a sustainable care more easily; and the caregiver may become more efficient, more powerful and can be more conscious; and the nurses also will be more convenient, more comfortable and more flexible and can perform in all their roles.
